If an OTC item ordered online at HorizonExtraBenefits.com, by phone at 1-800-480-6598 (TTY 711), or by mailing us an order form is damaged during shipment, it will be exchanged or replaced for an identical item at no cost to you. Your new shipment will arrive within 7-10 days from when we are notified. Please notify us by calling 1-800-480-6598 (TTY 711).
