If your card is lost or stolen, please call the Horizon EXTRA Benefits Card Member Services line at 1-800-480-6598 (TTY 711) and select option 3. You can also visit HorizonExtraBenefits.com to report it and request a replacement. Once you receive your new card, you will need to activate it.
There are 3 ways to activate your card. You must activate your new card before use.
- Call 1-844-210-2175 (TTY 711). You will need your card number and expiration date to activate your card using our interactive voice response system, and you will be asked to create a PIN.
- Visit HorizonExtraBenefits.com and select Activate Card on the dropdown that appears when you select My Account located on the top right of the browser. You will be asked to enter the last 4 digits of the card number and to create a PIN.
- Open the myTotal Benefits Mobile App and select Activate Card on the pop-up screen that appears when you select the menu at the top right of the phone screen. You will be asked to enter the last 4 digits of the card number and to create a PIN.
Once activated the card may be used for purchases as a credit transaction. No PIN is needed when the card is used for a credit transaction. If you choose the debit card transaction option, you will need to enter the PIN created when activating the card.
