The Health Risk Assessment reward is added to your account upon completion of the assessment, this can take up to 72 hours to appear in your account. For all other rewards, visit HorizonExtraBenefits.com to fill out and submit a Health Screening Attestation form. You can also download the form and mail it in.
For assistance completing the form, call the Horizon EXTRA Benefits Card Member Services line Monday – Friday from 8 a.m. to 8 p.m. ET at 1-800-480-6598 (TTY 711). Once confirmed, your rewards will be automatically loaded onto your Horizon EXTRA Benefits Card.
